What is nurse-led home care?
Nurse-led home care means a registered nurse is responsible for your care at home, from the first visit onwards. They assess what you need, write the care plan, and keep checking that it is working as your needs change.
For families in Hornsby, that is the difference between a carer who simply turns up and a team that actually understands the health side of things. Wounds, medications, recovery after a fall, the early signs something is not right. A nurse notices, and acts.
We are locally owned and we keep our team small on purpose. You see familiar faces, your nurse knows your story, and help is close by when you need it.

Why families across Hornsby choose us
We keep our team small and our standards high. Here is what that means for your family.
Do you provide nurses or just carers?
Both, and a registered nurse leads the care. Our carers handle day-to-day support, while a nurse oversees the plan, does the clinical care, and steps in when health needs change.
Will my parent see the same people each visit?
Wherever we can, yes. We build a small, consistent team around each client so the people coming into the home are familiar, trusted faces.
Can you help us work out funding?
Yes. We will sit down and explain Home Care Packages, Support at Home and private options in plain English, and help you use what you are entitled to. No pressure, no jargon.

Common questions
A few of the things families ask us first.
-
How much does home care cost in Hornsby?
It depends on how your care is funded. Many clients pay little or nothing out of pocket using a Home Care Package or Support at Home funding. Private care is charged at a clear hourly rate we quote upfront. We always give you the costs in writing before anything starts.
-
How quickly can care start?
Often within a few days for private care, and as soon as your funding is ready if you are using a package. If you are in hospital now, we can start planning straight away so you come home to support already in place.
-
Do I need a Home Care Package to use you?
No. You can pay privately and start whenever you like. If you do have a package, or are waiting for one, we can work with that too.
